Social and Community Support service

Voluntary Out Of Home Care (VOOHC)
At Unity Care, our Voluntary Out of Home Care (VOOHC) services provide a safe and supportive environment for children and young people who need temporary care outside their usual home.
Our dedicated team works closely with families to ensure that each child receives personalised support tailored to their specific needs.
We focus on creating a nurturing atmosphere that promotes well-being, stability, and development, ensuring that every child feels valued and supported during their time with us.
Through our VOOHC services, we aim to provide a seamless and reassuring experience for both children and their families.
Supported Accommodation
Supported Independent Living (SIL)
At Unity Care, our Supported Independent Living (SIL) services are designed to empower individuals with disabilities to live as independently as possible.
SIL funding can be used to cover the cost of support workers or other forms of assistance, helping individuals with daily living tasks, community participation, and skill development.
We offer SIL in shared accommodation settings, where multiple people with disabilities live together and share support services, fostering a sense of community and promoting social inclusion
Specialist Disability Accommodation (SDA)
At Unity Care, our Specialist Disability Accommodation (SDA) services are tailored for individuals with significant disabilities and very high support needs.
SDA funding covers the cost of specially designed or modified accommodations that include accessibility features and assistive technologies, enabling residents to live more independently and safely.
This funding is separate from other NDIS supports and is available exclusively to eligible participants with extreme functional impairments or very high support needs.
Our SDA services ensure a supportive and safe living environment, promoting greater independence and enhancing the quality of life for our residents.
Short- Term and Medium-Term Accommodation (STA & MTA)
At Unity Care, we provide both Short-Term Accommodation (STA) and Medium-Term Accommodation (MTA) services to support individuals with disabilities and their families during times of transition or when extended support is needed.
STA offers a temporary solution when usual caregivers need a break or during specific situations such as periods of crisis or transition.
This support can be provided in various settings, including respite centres or other accommodation options. MTA, on the other hand, is suitable for individuals who require extended support, such as transitioning from hospital to home or during other significant changes in their circumstances.
MTA provides the necessary support while participants and their networks plan for longer-term accommodation solutions, ensuring a seamless and supportive transition.
Individualised Living Options (ILO)
Is a flexible support package offered through the National Disability Insurance Scheme (NDIS) in Australia. It is designed to help people with disabilities explore and implement different living arrangements that best suit their personal circumstances, preferences, and support needs.
ILO is centred on providing participants with more choice and control over where and with whom they live. Unlike traditional Supported Independent Living (SIL) arrangements, which often involve shared housing with formal support, ILO offers various options that can be customised. These include:
Living Alone
Participants can receive supports to live independently in their own home.
Living with Family or Friends
ILO can be used to support individuals to live with family members, friends, or other trusted people, with customized support in place.
Living with a Host or Housemate
A participant can live with a host or a paid or unpaid housemate who provides support and companionship.
Co-Residency
Participants may live with a person who offers in-home support, tailored to their needs.
The ILO support framework is developed in two stages:
Stage 1 - Exploration and Design
This phase helps participants explore their living options, consider preferences, and create a plan that reflects their desired lifestyle.
Stage 2 - Implementation and Monitoring
Once the plan is approved, participants receive the support needed to put their chosen living arrangement into action. This includes ongoing monitoring to ensure that the setup continues to meet the participant's needs.
ILO is ideal for individuals who want more personalized living choices beyond group home settings and prefer arrangements that provide greater independence and connection to the community.
